Depending on the condition of your skin, your provider may recommend deeper ablative treatment over non-ablative treatment. Essentially, the ablative treatment removes some of the outer layers of skin, while non-ablative passes through the skin without removing layers. For ablative procedures, your provider may recommend local anesthesia for comfort. Non-ablative treatment generally only requires topical numbing cream to combat the sensation.

  • Cut out unhealthy habits. If you’re a smoker, it’s in your best interest to stop for two weeks before and after the procedure. Smoking can inhibit your skin’s ability to heal in an efficient time frame.
  • Stop taking medications and supplements prior to your treatment. Before skin resurfacing, your provider will recommend that you stop taking aspirin, ibuprofen, vitamin E or other medications and vitamins that could impact blood clotting, at least 10 days before your treatment. 
  • Take your recommended prescriptions. Your provider may prescribe an antibiotic to take before your treatment as a bacterial infection preventative. 
  • Avoid tanning and too much sun exposure. Heavy exposure to the sun can impact how the lasers work on your skin. Stop tanning and stay out of the sun for long periods of time for about a month before your treatment.
  • Don’t get other facial treatments (unless advised by your provider). Strong chemical peels, dermabrasion and other deep facial procedures can also impact your skin resurfacing treatment. Avoid these additional treatments for about a month before your appointment.
  • Make your provider aware of your medical history. If you have a history of oral cold sores or shingles near the targeted treatment areas, you need to let your provider know ahead of the treatment.

Doing a breast lift, or any other body contouring surgery, on a patient who has lost a tremendous amount of weight is like watching a caterpillar cocoon blossom into a beautiful butterfly. At least that’s how Burr von Maur, MD, FACS, of CosmetiCare Plastic Surgery and Medspa in Corona Del Mar, Calif, describes it.

Maur completes postbariatric body contouring surgery several times a week at CosmetiCare. In his 25 years of experience, Maur has found that patients who undergo a body contouring procedure after weight loss gain a new mind-set. These patients see an increase in confidence that facilitates daily exercise and social engagement, including their participation in sports activities and hitting the beach for a swim. Seeing their confidence levels elevate is why he enjoys doing it.

“One of the main reasons I like to do body contouring on patients after their experience with weight loss is because these are some of the happiest patients! It is such a relief for them because they have been thinking about getting some work done for a very long time. When they finally do, it’s transformative,” he says.

But there is plenty to consider before moving forward, and these procedures are generally done in stages to receive the best results. Maur starts his consultation process by finding out what bothers any given patient the most. Some patients want to get rid of the hanging skin on their tummy first, while others opt for an entire lower-body lift (tummy tuck, thigh, and buttock lift) all at once. A breast lift procedure can be done at the same time, but it will add more time to surgery (unless you have a second team to do the work).

Losing a lot of weight quickly may also result in large amounts of excess skin on the back and under the bra line. So the second stage of the body contouring process is an upper body lift, which can also be combined with the breast lift in one procedure.

“If the breast bothers a patient the most, then that’s what we do first,” he says. The volume of breast skin on a patient is sometimes severely stretched due to the weight loss. For those cases, Maur uses the GalaFLEX Surgical Scaffold to support the breast tissue during an implant.

Whether to improve everything at once, or take each procedure step-by-step, will mainly depend on the patient’s general health, motivation, and their mobility. Patients who have continued obesity, high body mass index, diabetes, uncontrolled high blood pressure, or significant heart disease are not good candidates for surgery.

“But if a patient is basically healthy and does not have any of those issues, then most of them do pretty well,” Maur says.

Maur requires every patient to undergo a complete screening (including a stress test) from their cardiologist and internist to make sure they are in satisfactory condition for surgery.

Maur is inspired by the late Ted Lockwood, MD. When the pioneering aesthetic plastic surgeon from Kansas City began presenting his early concept of the lower body lift to doctors across the country many years ago, Maur couldn’t wait to start training on the new method. Not many practitioners were getting involved back then, he says. But today, it’s becoming more and more popular among plastic surgeons, which Maur says is because it’s a rewarding procedure to do.

“I’m really fortunate to have become involved with this early on because I’ve grown with it. I have a strong-wide background—having done it for 25 years,” he says. “I know what my limitations are, and I know what realistic expectations can be for myself and the patient. That’s important.”

There is an educational curve that is required to treat a patient who has experienced significant weight loss. For plastic surgeons who are interested in expanding their services to specialize in body contouring after weight loss, he recommends lectures and seminars that focus on the various techniques, particularly since so much of it continues to evolve. Then you should select the one procedure with which you feel the most comfortable.

To promote this specialized procedure to patients, Maur suggests external and internal marketing campaigns, as well as having staff members talk with patients about it.  He says it’s also helpful to liaise with bariatric surgeons in your area. That’s a good way to not only obtain new patients but to also have a better understanding of all the nuances that are tied to the disease of obesity, he says.

“I’ve learned a lot about obesity throughout my experience over the years, and it’s more than just a psychological inability to control your eating,” he notes. “It’s often difficult to control without surgical intervention. And there is a physiological cause of obesity that is related to social, hormonal, psychological imbalance.”

As obesity rates continue to rise in the United States, Maur predicts that as people become more educated about body contouring treatments after weight loss, the trend will continue to grow.

The Real Housewives of Orange County mom’s pre-reunion treatment looked pretty scary, so we got an expert to give us all the details.

Apparently, if you want to look like a million bucks you have to spend it — or at least $2,500. Shannon Storms Beador has been an open book with fans on how she prepared for The Real Housewives of Orange County reunion taping this year, and one of those steps was a CO2 Laser Facial at Cosmeticare.

Two weeks after her procedure, Shannon shared some of the progress pictures of what her skin looked like immediately following the facial each day for the first week, as well as a picture at the two week mark. Shannon’s face was completely red and raw that first week, but 14 days later, it looked incredible! We had to learn more about this facial that looks so extreme at first, but made such a big difference after healing.

BravoTV.com chatted with aesthetic registered nurse Tannaz Razavi from CosmetiCare, (who also performed Shannon’s O-Shot) for the scoop on the CO2 Laser Facial. Read on to discover how it works and who is the wrong candidate for this procedure.

What is the CO2 laser facial?

Tannaz Razavi: CO2RE Laser Fractional Skin Resurfacing is an incredible treatment for select skin types that could benefit from correction of textural and pigmentation irregularities. The primary focus area of fractional resurfacing treatments at CosmetiCare is a full-face or partial face treatment, and this procedure can also be performed on neck, chest, or hands.

How does it work?

The laser causes micro injuries in the skin to create new tissue and collagen. Superficial, medium, or deep penetration may be performed to improve damaged skin, depending on the individual.

How is it different than other types of facials?

The CO2RE is more than a facial, it’s a laser skin resurfacing. With the typical facial or facial peels, you might experience light peeling; with the CO2RE you are having dermal skin resurfacing stimulated by a laser that’s stimulating collagen, resulting in tighter, brighter, and more beautiful skin.

How much does it cost?

The average treatment is $2,000 for the full face. The treatment Shannon received included PRP (Plasma Rich Platelets), which aid in faster recovery. This was important with her schedule and to get her reunion ready! With the PRP add-on, the average treatment cost is $2,500.

How often do you need to get it?

That all depends on the patient’s skin. If they have textural issues, it may take more than one treatment. Typically, once or twice a year is enough to keep collagen stimulated and skin tight and looking its best.

What are the risks for this type of facial?

In certain skin types, there is a risk of developing a temporary or permanent color change in the skin. Taking birth control pills, pregnancy, or a family history of brownish discoloration on the face may increase the possibility of developing abnormal pigmentation, which is why this treatment is recommended for select patients. Before having the treatments, it’s important for a patient to have a consultation with an experienced and licensed provider and disclose any history of keloids, unusual scarring tendencies, extensive X-rays or radiation to the face, or recurring cold sores (because this treatment can exacerbate complications).

How long is the recovery for it?

A reaction that feels and looks like a sunburn occurs following the treatment. The treatment usually involves redness and swelling, followed by aggressive peeling that ends within seven to 10 days. Deeper resurfacing includes scabbing with additional days of redness and swelling. It is important to avoid overexposure to the sun and exercise immediately after a treatment, since the new skin is fragile and more susceptible to injury and infection.

Who is a good or bad candidate?

A good candidate is any individual with a lighter skin tone that has fine lines, wrinkles, and/or sun damaged skin. In Orange County, where our practice is located, many of our patients spend a great amount of time outdoors, laying out by the beach and in the sun. This is a great treatment for many of those patients! The patients that I would not complete this treatment on are those with autoimmune disorders or hyperpigmentation conditions like melasma or darker skin tones. For these patients, we have another option that’s just as aggressive, but with less risk of adverse reactions.

Anything to add on the subject?

For those who are experiencing skin laxity, I would recommend adding microneedling with radio frequency, like Shannon had prior to the CO2RE treatment and Kelly had before the reunion. Specifically, the Morpheus8 treatment, as it provides excellent skin tightening results that complement our CO2RE laser facial.
